By twist all I meant waas that by the end it's unambiguous that the clinic story was a dream (unless Sarah really is dead and the rest of the series is a dream). I agree with selenaK's interpretations that the hostage scenes with the guy she killed are meant to be real (http://selenak.livejournal.com/460926.html) on the basis that Sarah's hands and mouth are bloody in her final scenes. if that's true the what happened is that Sarah went to check out the metal company, got tasered by Wilson/Winston, had the first of her clinic dreams while unconscious from that and the subsequent ones at the various points when she was drugged or knocked out during the interrogation. Alternatively the scenes with Wilson/Winston are also hallucinations/dreams although there's no framing device with a 3rd reality to back that up. Psychologically though, it works either way.
